Fernandes earned the award after an impressive campaign in which he matched the record for the most assists in a single Premier League season with 20, equalling Thierry Henry and Kevin De Bruyne.
The 31-year-old midfielder has also scored eight league goals this season, helping Manchester United secure Champions League qualification.
Fernandes becomes the first Manchester United player to win the award since Nemanja Matic in the 2010/11 season.
He beat competition from Arsenal trio David Raya, Gabriel Magalhaes and Declan Rice, Manchester City striker Erling Haaland, Brentford pair Antoine Semenyo and Igor Thiago, and Nottingham Forest midfielder Morgan Gibbs-White.
The Portuguese midfielder had earlier been named Football Writers’ Association Footballer of the Year.
Meanwhile, Manchester City defender Nico O’Reilly won the Premier League Young Player of the Season award.
The 21-year-old established himself as a regular for City this season, featuring mainly at left-back while also playing in midfield.
O’Reilly scored five goals and provided three assists during the campaign.
He also scored both goals in Manchester City’s League Cup final victory over Arsenal in March and featured in their FA Cup final triumph over Chelsea.
Other nominees for the Young Player award included City’s Rayan Cherki, West Ham’s Mateus Fernandes, Newcastle United’s Lewis Hall, Brentford’s Michael Kayode, Manchester United’s Kobbie Mainoo, and Bournemouth duo Alex Scott and Eli Junior Kroupi.
